Am I eligible to receive home visiting services from TLC Family Resource Center?
If you are expecting a child or have a child and live in Sullivan or lower Grafton County, New Hampshire you are eligible to receive services. Simply give us a call at (603) 542-1848 or stop by our offices at 109 Pleasant St. in Claremont, NH.
Do I have to pay for your services?
All services may be free. TLC Family Resource Center may bill your health insurance for the services we provide. If you don’t have health insurance, grant funding is available to cover the cost of services. No family is turned away due to inability to pay.
Do I need a referral from my doctor to enroll in your program?
We do not require a referral from your doctor but your health insurance company may require a physician order.
Why should I enroll in a TLC Family Resource Center program?
Raising children is complicated, stressful and challenging for all mothers and fathers, and every parent can use a helping hand at times. We provide families with information, guidance and support to help parents have healthy babies and to raise healthy, happy children.
What kind of training does the staff have?
The TLC Family Resource Center team consists of registered nurses, certified parent educators, and licensed mental health workers. For pregnant and postpartum women, a registered nurse and a parent educator each meet with the family on an alternating basis throughout pregnancy, continuing until the child is up to three years old, depending on the program in which they are enrolled.
How often do you visit?
The schedule depends on the needs of the family. Visits usually take place on a weekly or bi-weekly basis for a limited number of weeks. Most visits are one hour. For pregnant women, visits increase around the time of birth. Women who enroll during pregnancy receive visits until the child is up to 3 years old.
Do visits have to take place in my home?
No. Visits outside of the home can be arranged for those who prefer a location outside of the home. Sometimes it is easier for families to meet at the TLC Family Resource Center office in Claremont or at another location in the community. Just let us know.
What happens during a visit?
The visits are designed to help families reach their goals. Each visit includes time for listening to parents’ concerns, to offer information and support, and to review progress on family goals. For example, if the goal is to learn about child development, the parent educator will provide age-specific information about the physical, cognitive, social and emotional development of the baby or child. They might offer a book or game suited to the age of the child. Sometimes the parent educator will use a screening tool called the Ages and Stages, which helps to screen for developmental strengths and areas in need of improvement.
Are the services you provide confidential?
Yes. Please see our notice of privacy practices. A signed release form must be on file in our office before information about you or family may be shared, with few exceptions.
