INDIVIDUAL FAMILY SUPPORT

TLC has Family Support Specialists and a Nurse who can visit you at home, in the community, or in our family-friendly offices to explore and support family goals and healthy child development.  

Home Visiting Services

Home Visiting services are available to pregnant moms, and to families with children up to 21 years old.  They may take place in the convenience of the home, TLC’s office, in the community or via Zoom.  Home Visiting Services help parents develop the skills, knowledge, and resources needed to raise a healthy and happy child and may focus on:

• Health and immunizations
• Parent and child attachment
• Child development
• School readiness and achievement
• A safe and nurturing environment
• Family and parental resiliency
• Community resources and supports
• Setting and achieving goals
• Family economic self-sufficiency and more

kinship navigator services

If you are providing a child with a kinship arrangement, which includes adoption, DCYF placements, foster care, legal guardianships, and informal guardianship, let our Kinship Navigator help connect you to:

• Services and support that meet the needs of both children and caregiver
• Emotional support 
• Education and guidance 
• Information and referral
• Follow-up services to promote independence and enhance the well-being of the family

POSTPARTUm SUPPORT INTERNATIONAL

PSI Coordinators provide caring and informed support and resources to moms and their families.  TLC hosts the PSI program at our Claremont and Newport locations.

For mothers who need help with breastfeeding, offered in-home or at TLC.
